Do you know a friend, neighbour or relative who would benefit from the services in the Wellness Centre’s services for caregivers? If so, please send them this page, and suggest that they use our modifiable registration form so they can confidentially sign up for support.
Registering will provide community members with access to our caregivers’ concierge service, lending library, monthly e-newsletter, as well as reminders about our Well 50+ & Caregivers events and other community outreach activities.
Once you have completed the form
After you have filled in the registration form, don’t forget to save it to your computer. Send it as an attachment by email to Anaïs, our Well 50+ Caregivers’ Program Coordinator, at: afortinmaltais@jhpartners.net
